Marist vaccination event is August 11

Walgreens will be at Marist High School on Wednesday, August 11 from 3 to 7 PM for the first shot of the Pfizer vaccine.

Please review the Pfizer COVID-19 Emergency Use Authorization (EUA) Fact Sheet
Please read and fill out the Vaccine Administration Record (VAR)
Parents must fill in the phone number field of Section A of the VAR form
Parents must sign the VAR form (Section C) to confirm consent

Students must bring the VAR and their insurance card to their appointment. Parents do NOT need to accompany their child if the child has a VAR signed by the parent with a parent phone number in the phone number field.

If you would like to register for the vaccine, please email oneill.beth@marist.net and confirm that you will be attending. You may come anytime between 3 PM and 7 PM….you do not need to make an appointment for a specific time. Information regarding the date of the 2^nd shot will be provided at the event on August 11.
